The country was in the process of breaking up at the time, with lots of confusion, and there was hyper-inflation to handle - I suspect that keeping a detailed record of such things might not have been considered a priority.

I can see it is listed on Wikipedia as "P.142A 100,000 Dinara 1994" and can get 2 or 3 images of it but their is no actual issue date? And I cant actually find one online bar referances to it.
