Hopefully your uncle was smart enough to buy the stock in a custodian account for you and the stock was delivered to you or to the custodian .

If the stock is in the name of a custodian for you, just send the stock certificate to the transfer agent (the name is on the front of the certificate) along with a copy of your birth certificate. Send a letter request the stock be registered in your name

If the stock is only in your name - you may be screwed, call the transfer agent and se what other papers they may need along with the birth certificate. Have them register the stock in your name

If the stock is held by a bank and/or a broker/dealer, take the stock to the firm along with a birth certificate and they can handle it for you
